Case Manager – Community Services (RNLD)

Location:Birmingham, Wolverhampton, Coventry, Stafford, Stoke-on-Trent, Manchester, Nottingham, Leeds & Sheffield. South West (Dorset, Cornwall, Somerset, Devon, Wales, Gloucestershire, Wiltshire)

Key Responsibilities:

  • To support the MedGen Community team with service delivery of care packages.

  • Manage and maintain a caseload of clients within the community.

  • To build positive relationships with the clients and their family’s or wider networks.

  • To thoroughly understand the clients and family’s needs and ensure the abilities of the staff working within the package match the care plan.

  • Support new members of staff to be introduced to the package of care through meet and greet with service user and families.

  • To be the lead person responsible for shadow shifts and training shifts.

  • To role model, supervise and assess staff competencies and build capability through coaching.

  • To identify learning needs within the package and communicate to the appropriate teams.

  • To fully support with any investigations following an incident or a complaint.

  • To identify and report environmental and personal changes in the home environment which may be seen as a risk to both staff and the client.

  • Effectively communicate with the Registered Manager and Clinical Lead on a regular basis to ensure good management, quality and safety within the package.

  • Support the wider team to ensure all shifts within the package are appropriately covered and to provide emergency cover when required.

  • To complete weekly PPE stock takes to ensure stock levels remain at a satisfactory level.

What We’re Looking For:

  • RNLD qualification with current NMC registration

  • Minimum 2 years’ experience in community care or a similar setting

  • Coaching experience (Train the Trainer qualification preferred)

  • Strong understanding of CQC Regulations and Social Care Codes of Practice

  • Confident with IT systems and documentation

  • A proactive team player with excellent relationship-building skills

  • Full UK driving license and willingness to travel within the South West region
